Form 1095-C: Notice of Availability
This notice is intended to provide Anika Therapeutics Medical Plan participants with instructions to request a copy of form 1095-C for tax years 2025 and forward under the new requirements set forth by the Paperwork Reduction Act (update December 2024). Forms will be provided upon request.
Anika prepares and files individual forms 1095-C: Employer-Provided Health Insurance Offer and Coverage annually for each employee within the tax year. Form 1095-C: Employer-Provided Health Insurance Offer and Coverage is a tax form used to report information about an employee’s health coverage and also provides monthly enrollment information. This form is not required to be filed with a federal tax return, and employees may keep this form for their records. Information from this form can also be used to help determine a taxpayer’s eligibility for credits that may be available as they relate to affordable care.
To request a copy of Form 1095-C, please contact Anika Human Resources (HR) via the information below. Anika will provide a copy of 1095-C forms within 30 days of request via US mail or electronically.
