Effective immediately, please be advised that all orders and event requests require approval before making any commitments. While submission of this form is necessary, it does not automatically signify approval for the respective event. We will reach out to inform you of the status of your event once it has been reviewed and approved.

Local Bio-Skills Lab Request

Anika Medical Education will receive your request. Once vetted and approved by Medical Education and executive management, Medical Education will work with the region to coordinate a local bio-skills lab between the Health Care Provider (HCP) and regional team. If a HCP proctor is needed for this lab please provide details and ensure approval is confirmed.

Please ensure all known distributor labs are also being requested for tracking purposes.

If your lab requires Anika Sports Medicine implant samples and/or Arthrosurface lab instrumentation please ensure the requestor has filled out an Anika sample and demo request form following the local lab request. It is the responsibility of the requestor to provide an accurate implant sample request to Molly Kelley AND James Hurkett and this is done through the Sample Request Form.

Local Lab Details:

  • Request minimum two (2) weeks in advance
  • Request minimum four (4) weeks in advance if HCP consultant proctor required
  • Supported by Anika Medical Education
  • Executed by the local Anika Commercial Team and additional Anika employees as needed. 
  • Medical education works with region to determine learning objectives, content and support required. 
  • Half (1/2) day event
  • Lab Average Time = three (3) hours
  • Specimens ordered by Anika Medical Education based on accurate lab request info
  • Up to fifteen (15) surgeon attendees
  • Hands on cadaver lab or sawbones demo with experienced Anika representatives
  • May include surgeon speakers and HCP lab proctors
  • One (1) commercial rep per surgeon attendee
  • Requires approval from their Anika DOS prior to submission
  • Distributor travel and accommodations are funded by the distributor.

Surgeon to Surgeon (S2S) Discussion Request

Info:

Anika Medical Education will receive your request. Once vetted and approved by Medical Education and executive management, Medical Education will coordinate a video chat on MS TEAMS or phone discussion between the Health Care Provider (HCP) and an Anika educational consultant. 

Surgeon to Surgeon (S2S) Details:

  • Requested minimum 1 week out
  • Sharing of Knowledge, Evidence and Clinical Practice
  • Executed by Medical Education
  • Include required details to assist with setting up the discussion.
  • Include required dates and times of HCP availability.
  • Med Ed will connect the HCP to an educational consultant via MS TEAMS / Audio
  • Executed as soon as possible (depending on the day of request and faculty availability)

Anika Innovation Symposium Request

Anika Medical Education will receive your request. Once vetted and approved by Medical Education and executive management, Medical Education will work with the region to coordinate a Innovation Symposium between the Health Care Provider and a Anika educational consultant. Anika Med Ed organizes the surgeon speaker and works closely with region to tailor event details to meet the attendee learners needs.

Innovation Symposium Details:

  • Request a minimum of four (4) weeks in advance
  • Supported by Anika Medical Education
  • Executed by the local Anika Commercial Team and additional Anika employees as needed
  • Subject to regional and national KOLs availability
  • Commercial is responsible for organizing dinner venue within Anika policy and AdvaMed guidelines.
  • Half Day (1/2) Event – Evening, Journal Clubs, or Grand Rounds
  • Average Time = 3 hrs
  • Up to fifteen (15) surgeon attendees
  • Anika Faculty Consultants Discuss Products, Procedures, Recent Evidence and Patient Cases
  • Sawbones demonstrations encouraged with implants and instrumentation
  • One (1) commercial rep per surgeon attendee
  • Requires approval from Anika DOS and Medical Education
  • Distributor travel and accommodations are funded by the distributor.
  • Meal limit is set at $160 USD after tip and tax. Regional teams are required to follow these guidelines. Submission of receipt through ConCur following event required with HCP information. 
  • Name and signatures of all HCPs attending event must be documented.

Anika Visiting Surgeon Program Request

Anika Medical Education will receive your request. Once vetted and approved by Medical Education and executive management, Medical Education will work with the region to coordinate a VSP between the Health Care Provider and a Anika educational consultant. Anika Med Ed organizes the visit and event and works closely with region to tailor event details to meet the attendee learners needs.

Important:

  • The form below is intended to request to have a HCP attendees participate in Anika’s Visiting Surgeon Program (VSP).
  • This is subject to availability of the consultant and will be organized once a time is available and the surgeon has a case scheduled.
  • A typical VSP will include two (2) case observations.
  • VSPs should be organized and communicated by Anika Medical Education.
  • Every effort should be made to ensure that the HCP attendee and Anika faculty have not communicated about this event and visit prior to organizing with Anika Medical Education.

Visiting Surgeon Program Details:

  • Anika has nine (9) sites for OVOMotion and one (1) OVO. Dr. Chris Baker and Dr. Wasik Ashraf will also include SPM and TACTOSET procedures.
  • Subject to regional and national KOLs availability
  • Half Day or Full Day of educational observation in the Anika Faculty operating room
  • Includes 30 min phone conversation with consultant prior to HCP attendee’s scheduled case. 
  • Educational discussions on Anika products over lunch and/or a dinner following observation or night prior
  • Anika Faculty Consultants Discuss Products, Procedures, Recent Evidence and Cases
  • Requires approval from Anika DOS and Medical Education
